This guide is for all users who place Pathology orders or collect specimens. The back up forms are used only if there is a Trust-wide failure of Label Printers or if Sunrise goes down completely

Fall Back Forms |

As Biochemistry and Haematology orders do not generate a printed order form, they rely on the specimen label and the electronic message to relay the information to the Pathology lab. Fall back forms are used for these orders only.

Fall back forms are used on the wards, when the label printers go down. IT switch on the printed order forms (this could be site specific or Trust-wide, not a single label printer not working)

From the Clinicians’ perspective: The requesting clinician will use the ‘Add Specimen’ button to print the Biochemistry and Haematology order forms, which then go into the Phlebotomy tray in the ward with all the other order forms.

From the Phlebotomists’ perspective: The Phlebotomists will collect all the forms from their tray. They will handwrite the date and time of collection onto both the fall back form and the specimen container and process the orders as normal.

Note – there is no need for fall back forms for any other type of Pathology order, as they print the order form automatically.

Downtime Forms |

Downtime forms are for when Sunrise EPR goes down and no orders can be placed online.

Downtime forms are the paper order forms that are used currently. The date and time of collection must be handwritten onto the order form and the specimen container (following the normal paper processes).

Note – If the date and time of collection is not recorded, either on ‘Add Specimen and Printed label’ or ‘handwritten on the order form and container label’, the time in Sunrise will be recorded as 00:00 (midnight) and say ‘No Collection’. The results of these will not appear on the graph view in the correct position
