These supporting materials are for all users who collect specimens and samples. This may be collecting a sample at the same time as placing an order or when using an existing order.

Please read the following important messages before collecting samples:

 

Pathology Order Forms and Labels

 

  • The request is sent electronically to Telepath for Biochemistry, Haematology and Microbiology orders when the sample is collected.

 

  • Haematology and Biochemistry – no order form is required. The labels will print when the sample is collected. 
    • In the event of a Trust-wide issue with the zebra specimen label printers, IT will activate the fall-back forms. This means that, for Inpatients only, the requesting clinician is to click Add Specimen to automatically print order forms for Biochemistry and Haematology tests. The order form can then be placed 

in the tray for the Phlebotomists to collect the samples

 

  • The order form for Add-On tests will print in the Lab automatically. No specimens need to be collected

 

  • Microbiology and Cell Path Non-Gynae Cytology – the order form is printed when the order is placed on the ward but for outpatients it prints when the sample is collected. The specimen label is always printed when the sample is collected

 

  • Cell Path Histology – the whole process to remain on paper – order forms, labels and results 

 

  • Blood Transfusion - Cross Match and Group & Save and other tests – the order form is printed when the order is placed on the ward but for outpatients it prints when the sample is collected. Specimen labels will remain handwritten. The report will come back into Sunrise as an unsolicited result

 

  • Blood Product Issue – no sample needed, the order form prints in the Lab, always ring the Lab for this to be actioned. The report will come back into Sunrise as an unsolicited result

 

  • Routine orders will have an “R” printed on the label and Urgent orders will have a “U”

 

  • Paediatrics and neonatal samples - the small container will be within a larger container with the label on the outer. Paediatric volumes for ages 2 years and under are displayed on blood labels. Clinicians who collect paediatric samples will be aware of the volume required

 

  • If specimens are not collected within 12 months of the due date, the order will automatically be closed

 

  • Any unused labels to be put into the sample bag so they can be disposed of correctly at the Lab

 

  • Results of GP requested tests will be available to view as unsolicited results in the 
     ‘All Available Charts’ option

 

 

  • Outpatient ‘collect at home samples’ – the nurse will click Add Specimen on Sunrise to print the order form and specimen label, which will then be attached to the container and given to the patient. The patient will write the date and time of collection on the order form

 

  • When an order is placed in Outpatients, or on the ward where the patient will be returning as an outpatient for the specimen collection, the patient will be given a card with the required option ticked (see below)

 

 

When to use Sunrise for collecting specimens

 

The Add Specimen process only applies to Pathology orders that have been placed on Sunrise. It does not apply to orders on paper order forms

 

The following areas will not be placing Pathology orders on Sunrise:

 

  • Histology
  • Theatres
  • Sexual Health Clinic
  • Occupational Health
  • Breast Screening Clinic 
  • Satellite Sites
  • Community Nurses
  • Community Midwives

 

Note: All Pathology orders from these areas will be placed on paper order forms and the existing process for specimen collection will continue

To collect the sample at the same time as placing the order, select the test from the list at the bottom of the ‘Order Entry Worksheet’ and click ‘Add Specimen’

With the patient in context in the blue banner, select the ‘Orders’ tab. Click the + to expand the ‘Pathology’ section to see all the tests

Tests marked for collection in the future will have the status of ‘Pending’ which will change to ‘Pending Collection’ 24 hours before the due date and time

Right click on the appropriate test and select ‘Add Specimen’.

 

 

 

 

 

 

 

 

 

Alternatively, highlight the test and click ‘Add Specimen’ on the toolbar

Note:

  • The ‘Collection Date and Time’ default to now but can be amended if required.
  • There is no need to adjust the ‘Number of Labels to Print’ – Sunrise knows how many labels to print for the tests selected
  • Labels for the specimen containers print on the mini label printers

The ‘Specimen Type’ field should default to the correct type but can be changed if necessary using the dropdown arrow.

 

Tick the tests you are collecting from the list. If collecting multiple samples from the ‘Specimen Type’, tick all tests that apply. 

 

Note: Only samples associated with the current visit will be displayed for collection

Add any notes to the ‘Additional Comments’ section. Click ‘OK’ to record the sample has been collected and to print the labels and/or order forms

 

 

 

 

 

 

 

 

Note:  If swabbing multiple sites ensure the site is handwritten on each label

If collecting the specimen on an existing order, the screen will return to the 'Orders’ tab (see Step 10). However, when the specimen is being collected whilst placing the order, the ‘Order Entry Worksheet’ will be displayed. The status of the test will now show as ‘Collected’.

 

To submit the order for all the tests, click ‘Submit Order(s)’ at the bottom of the ‘Order Entry Worksheet’

Note: If there is Trust-wide failure of label printers or if Sunrise goes down completely, please refer to the Quick Reference Guide (QRG) on ‘Backup Forms’
