This guide is to show the Therapy teams how to complete follow-up contacts on the Inpatient Daily Therapy Note (Adults and Paediatrics) and Data Collection Flowsheet in Sunrise.

Important Note:  Therapy teams will need to use the ‘Therapies Data Collection’ flowsheet to record their contact activity as soon as Sunrise goes live.   However, the therapy ‘documents’ can only be used when the patient’s ward has switched to doing their inpatient documents on Sunrise

Select the patient you require from your chosen ‘Patient List’ and check the details in the patient banner.

 

 

 

Alternatively, select the patient from the relevant ward ‘Tracking Board’

Every time the patient is seen, a new Daily Therapy Note will need to be completed.   Click on the ‘Enter Document’ icon in the main toolbar

The ‘Document Entry Worksheet’ pop-up window will open.      Ensure the date and time are correct and select ‘Start of Browse’ from the dropdown menu to display the folder choices.

Select the relevant document, i.e. ‘IP Daily Therapy Note’ for Adults or ‘Paediatric IP Daily Therapy Note’ for children.  To open either left click the document and press the ‘Open’ button below or double click the document to open.

The relevant Daily Therapy Note will be displayed.  A completed example can be seen in Step 7.   

 

As explained in the QRG ‘Therapies – First Contact’, the following sections should be completed by the Therapist:

 

 

  • Therapy Service – to choose the correct service
  • Refresh’ button  – to display the latest observations
  • Body Map            – for annotation
  • Therapy Notes    – to enter SOAP information for the patient        

 

 

Note:  If you need to copy or duplicate information from one document to another it is possible to do this via the ‘Copy Forward’ and ‘Copy and Paste’ function.  For further details see the Quick Reference Guide ‘Creating Documents – Key Points’

Once complete, click on the ‘Save’ button on the bottom right of the screen

The ‘IP Daily Therapy Note’ and ‘Paediatric IP Daily Therapy Note’ can be viewed in the ‘Documents’ tab.    If required, existing documents can be viewed and modified by double left clicking on the document. 

 

 

 

 

 

A pop-up window will display your chosen document and you can then select the Modify tab to open the document.  The scroll bar on the right can be used to move up and down.

Now click on the ‘Flowsheets’ tab to record your contact activity

In the ‘Options Panel’, under ‘Flowsheets Selection’, select the flowsheet ‘FS Therapies Data Collection’

The flowsheet will now be displayed in the centre of the screen.    Expand the ‘Follow Ups’ parameter by clicking on the ‘+’ icon

To enter information into the Flowsheet, click on the ‘Add Time Column Options’ icon from the tab toolbar

Select ‘Add Single Time Column’ from the menu options

The single time column pop-up screen will now appear, the current date and time are automatically entered, use the arrows at the sides if any changes are required, then click ‘OK’

The new date/time column will be added to the Flowsheet.

To complete the ‘Follow-Ups section, click into the new time/date column.     Individual pop-up windows will appear for each field.  Enter the appropriate data and then click ‘OK’ to move on to the next field.  

 

  • It is paramount the correct ‘Follow-Up Contact Therapy Team’ is selected, for example, ‘Physio - T&O - TWH’
  • Do not complete any fields that are marked for another therapy’s use, for example ‘Diet Type (Dietetics only) and ‘Discharge Outcome (Not for use by Physiotherapy) 
  • When recording the amount of time spent with a patient, you must type in the number of minutes.  Do NOT click the clock symbol.  For example, if the therapist spent 45 minutes with a patient they would fill in 00:45 or for 75 minutes it would be recorded as 01:15

Once you have completed your entry, click on the ‘Save’ button, in the top right hand side of the Flowsheet

 

 

Note:  This process can be repeated for every ‘follow-up’ contact, as seen in the image above.

At the final contact, expand the Outcomes parameter and enter into the required fields. This may be done at the same time as the ‘follow up’ entry is made and in the same time column.

Note: When more than one Therapy team is involved in the patient’s care, they will have their own documents but will share the same Therapies Data Collection Flowsheet.      For further details see Quick Reference Guide ‘Inpatient Therapies – Multi-Team Involvement’
