This guide is to show the Therapy Teams who may use the West Kent Health & Social Care Assessment Additional Document for a patient on Sunrise, on where to find the document and how it works.

Note – You are able to add your most frequently used documents to access all in one place, for easy use. For more details, please refer to the Quick Reference Guide ‘Set Favourite Documents and Default Display’.

Select the patient you require from the patient list and check the details in the patient banner.

Click on the ‘Enter Document’ icon on the main toolbar. This is how to start a new document.

With the Enter Document screen displayed, from the ‘Start of Browse’ list, showing all the document categories, select ‘Therapies’ to display the Therapy documents available on the right-hand side

Or you can alternatively use the search box by typing in ‘TH’

From the Therapies document list, double-click on the required document to open it.

 

Alternatively select the document and click the ‘Open’ button

With the TH West Kent Health and Social Care Assessment open in a separate window, all the available tabs will be displayed across the top of the screen

Some tabs may contain information which has pulled through from Inpatient Documents completed through the patients stay, this will be indicated by a ‘Stack of Books’ icon in the tab heading

Click on the tab(s) to view the information

Complete the necessary fields required, clicking on the different tabs.

Majority of the fields within the document are either free text boxes, selection check boxes and/or radio buttons.  Example below -

Within the ‘Health’ tab, there is a grey button ‘Seen by UCT?’, which the Urgent Care Team can use to add a referral to their team.

Within the ‘Function & Mobility’ tab, a lot of the sections are within two columns - ‘Pre-admission information’ and ‘On-assessment’. These boxes are free text fields for you to enter the data required.

Some questions have the ‘Yes’ and ‘No’ radio buttons, which will evolve more questions underneath depending on the answer marked.

Note – If you require to clear the data for a radio button question, right click on the radio button answered and select ‘Delete Data’ from the menu options

Once you have finished entering the details required, click the ‘Save’ button at the bottom of the document

 

If more information for this appointment is to be added later, ensure the ‘Incomplete’ check box is ticked in the bottom left corner before saving and the status of the document will show as ‘Incomplete’ on the Documents tab

To return to the TH West Kent Health and Social Care Assessment to continue entering information, with the patient in context, click on the ‘Documents’ tab

With the documents screen displayed, select the TH West Kent Health and Social Care Assessment, for the visit you require (check the date)

Either right click and select ‘Modify Document’

 

 

Or double-click on the document to open and click the ‘Modify’ tab

 

 

Or click on the ‘Modify’ icon from the tab toolbar

Continue data entry

If the Therapy Team feels that the patient needs to be referred to another care provider, click on the ‘Onward Referral’ tab and complete the section/s relevant

Once completed, ensure the check box for ‘Incomplete’ at the bottom of the document is unticked and then click the ‘Save’ button

 

 

The status of the document will show as ‘Complete’ on the Documents tab

To print the West Kent Health and Social Care Document as a report, with the patient still in context, click on the ‘Print Reports’ icon from the main toolbar

The ‘Report Selection’ pop-up screen will appear, from the Report Category dropdown box, select ‘Therapies’ and from the list displayed underneath, select the report you require.

Click on the ‘Preview’ button, to see the document before printing

With the document displayed in a separate window, check the information is complete and correct

Use the toolbar at the top of the screen to scroll through the pages and to print the report
