This guide is to show the Therapy Teams how to Triage a patient’s therapy referral and how to use the Data Collection Flowsheet to record their activity in Sunrise.

Note: Therapy teams will need to use the Therapies Data Collection Flowsheets to record Triage and contact activity as soon as Sunrise goes live.   

 

Note:  If the patient is transferred from one hospital site to the other, the patient will still appear on the original patient list if the referral is specific to the site.     The ‘Current Location’ column on the patient list will highlight this. For the patient to appear on the patient list for the new site, the original referral must be marked as ‘Complete’ (for the referral to drop off the patient list) and a new referral will need to be added for the new site.

Select the patient you require from the patient list and check the details in the patient banner.

Click on the ‘Orders’ tab

With the ‘Orders’ tab displayed, you can see the therapy referral under the ‘Referrals’ banner and this shows the referral order date and the status ‘Active’.     This example shows a ‘Physiotherapy’ referral

 

 

Important Note:   Any ‘Active’ therapy referrals that are not actioned will be automatically 'completed' on Sunrise after 4 weeks

Double left click on the referral to view the contents 

 

A pop-up window will display the relevant details for you to read.  Use the scroll bar on the right to move up and down the referral and when finished click the ‘Close’ button on the bottom right of the window.

Next, click on the ‘Documents’ tab

With the ‘Documents’ tab displayed, you can view any relevant Inpatient documents, such as ‘IP Doctor Ward Round’, IP Clinical Clerking Document’ or ‘IP Daily Therapy Note’.   To open, double left click on the document and a pop-up window will display your chosen document.    Note:  For further details see Quick Reference Guide ‘Documents Tab’

 

 

 

Important Note:  Therapy documents can only be viewed when the patient’s ward has switched to doing their inpatient documents on Sunrise

Click on the ‘Flowsheets’ tab

With the Flowsheets tab displayed, check the criteria in the ‘Options Panel’ on the left of the screen.  

 

 

Note: ‘This Chart’ will automatically be selected and will show all clinical observations and data from the current attendance only. ‘All Available Charts’ will display the clinical observations and data from all attendances.

From the ‘Options Panel’, check the correct dates are displayed in the ‘Date Range’.  Click the arrow at the end of the ‘From’ field to see an option of ‘Start of this Chart’ or select the dates required

If you have used any of the filters in the ‘Options Panel’, you can tick ‘Retain Selections for Next Patient’ in order for Sunrise to remember your choices

In the ‘Options Panel’, under ‘Flowsheets Selection’, select the flowsheet  ‘FS Therapies Data Collection’

The Flowsheet will be displayed in the centre of the screen.     Note: The flowsheet parameters are subject to change, however, the process for entering data will remain the same

Expand the ‘Triage’ parameter by clicking on the ‘+’ icon

Note: When more than one Therapy team is involved in the patient’s care, they will have their own documents but will share the same Therapies Data Collection Flowsheet.      For further details see Quick Reference Guide ‘Inpatient Therapies – Multi-Team Involvement’

To enter information into the Flowsheet, click on the ‘Add Time Column Options’ icon from the tab toolbar

Select ‘Add Single Time Column’ from the menu options

The single time column pop-up screen appears, the current date and time are automatically entered, use the arrows to change the date and time if required, then click ‘OK’

The new date/time column will be added to the Flowsheet.

Click into the time/date column to complete the following Triage questions:

 

  • Referral Date/Time

This is currently a manual input field but will be automatically populated eventually.  Ensure the data entered here is the date/time the therapy referral was made and then press ‘OK’. Do NOT enter the time you received or picked up the referral. Note: This information can be obtained from either the paper referral or by viewing the referral on the ‘Orders’ tab (See Steps 3 and 4).     

 

  • Referral Source (UCT only)

To be completed by UCT only.  Press ‘OK’

 

 

 

  • Referral Status

Tick the relevant radio button to indicate if the referral is ‘Accepted’ or ‘Rejected’ and press ‘OK’

 

 

 

  • Therapy Team 

It is paramount to indicate the correct Therapy team and then press ‘OK’ 

 

 

 

  • Priority at Triage

Select the priority P1 – P5 as appropriate and press ‘OK’ 

 

 

 

  • Referral Reject Reason

If the referral is being rejected, indicate the reason and follow your department’s process to inform the referrer.

When you have completed your entry, click on the ‘Save’ button, at the top right hand side of the Flowsheet

Finally, return to ‘Patient List’ to triage the next patient
