This guide is to show the Therapy Teams how the Data Collection Flowsheet works with Multi Therapy Teams Involvement under a patient’s care in Sunrise.

Note – When more than one Therapy team are involved in the patient’s care, they will have their own documents but share the same Flowsheet

Select the patient you require from the patient list and check the details in the patient banner.

Click on the ‘Documents’ tab

With the Documents tab displayed, you can see under the ‘Document Name’ column, the IP Daily Therapy Note documents with the different therapy teams name assigned to the note
|

Click on the ‘Flowsheets’ tab
|

With the Flowsheets tab displayed, check in the ‘Options Panel’ on the left hand-side of the screen, that you have the required criteria needed.
In the ‘Flowsheets Selection’ area, select ‘FS Therapies Data Collection’

Ensure the chart selection and date range is correct
|

The Flowsheet will now display in the main screen with all the parameters closed. There are four parameters within the Therapies Data Collection Flowsheet – Triage, First Contact, Follow Ups and Outcomes. Click on the ‘plus’ icons to open each parameter
Use the scroll bar at the bottom of the screen if you need to view previous entries.

Each Therapy Team will add an entry into each parameter – Triage, First Contact, Follow Up Contacts and Outcomes, example below -

Important Note – As all Therapy Teams are sharing the same Flowsheet, it is paramount that the correct Therapy Team is added to each entry.

Due to the Therapy Teams coming in at different times in the day, it means that your Therapy Teams Data Entry column will not necessarily follow on, one after the other.
Ensure you are reviewing the correct Therapy Teams data on the Flowsheet from the previous contact, if required. |
