This guide is for all Clinical Nurse Specialists in inpatient wards. It provides an overview of completing the inpatient CNS document for adult patients on Sunrise.

Documents created can be viewed by all staff with the appropriate access right on Sunrise.

Patient Admitted in Adult Ward

From the ‘Tracking Boards’ tab, choose an adult ward from ‘Department’ list and select the required patient.

 

 

 

Note:  Some frequently used inpatient documents are accessible from the quick launch documents shortcut option. Other documents not listed can be created by selecting on the toolbar and searching for the document.

From the Tracking Boards tab, select the Quick Launch Doc(s) button at the bottom of the screen, using the arrow to the side  and choose ‘IP Clinical Nurse Specialist (CNS) Note’ from the list provided.

 

Alternatively, right mouse click on the patient, hover on ‘Quick Launch Documents’ and select the same document.

The IP Clinical Nurse Specialist (CNS) Note is to be completed by CNS for adult patients once admitted on the ward.

 

 

 – indicates mandatory field and must be completed before you can save.

 

Note – You are required to scroll further down with the scrollbar on the far right of the page to see other relevant fields to be completed.

When the IP Clinical Nurse Specialist (CNS) Note is opened, some information may carry across from other inpatient documents that have been completed for this patient, for example the Nursing Admission, Clerking Document or the Doctor Ward Round and Progress Notes. The information will be displayed in the fields in the top half of the document.

Conditional triggers occur when a selection is made on a radio button with ‘…’ at the end. A question answered on the document could trigger additional questions. The additional questions might appear on the side or underneath the current question.

 

 

 

 

 

 

Example – ‘Yes…’ triggers another question below.

 

Example – ‘No’ selection doesn’t trigger any further question.

Quick Action Button may appear in the document. An example is the ‘Refresh’ button. This inserts the latest recorded observations for the patient into the accompanying field

Once selected in the document, a full body image can be annotated using the toolbar directly above the image.

Ensure the ‘Outcome of Referral’ at the end of the document, is recorded

Once fully completed, select the ‘Save’ button. If the document is partly completed and not finished, tick  at the bottom of the screen before selecting the ‘Save’ button. 

This action will indicate to colleagues within the document tab that the document is in progress.

SCM Notices appear as pop-up alerts regarding information recorded or for actions required. For example, saving a document whilst mandatory fields haven’t been completed. Some examples are illustrated below. Staff are required to read the prompt and acknowledge or action appropriately.
