This guide is for Paediatric Clinicians who add and manage paediatric clinical observations as part of completing the ‘ED Triage’ document and ‘Paediatric IP Nursing Admission’ document, as well as through the ‘Flowsheets’ tab within Sunrise.

Documents created can be viewed by all staff with the appropriate access right on Sunrise.

The ‘FS Clinical Observation Paeds’ is an example of a flowsheet within Sunrise. It will be completed by nursing staff with the appropriate access right. 

 

The paediatric observation can be taken and recorded electronically on Sunrise as part of completing the ‘ED Triage’ document if patient was admitted within emergency department (ED).

 

After ED admission, if a patient is then admitted into a paediatric inpatient ward where observations are recorded electronically, previous electronic observation/s taken whilst in ED including other inpatient admissions can be seen within the flowsheets tab if chart selection option is set to ‘All available Chart’ and the date range specified covers the ED stay. 

 

Upon admission to the ward, the initial clinical observations for paediatric patients will be recorded on the ‘Paediatric IP Nursing Admission’ document. Subsequent clinical observations will then be entered on the appropriate flowsheet in the ‘Flowsheets’ tab on the patient record.

Patient Admitted in Emergency Department (ED)

From the ‘Tracking Boards’ tab, choose an ED location from ‘Department’ list, then select ‘Paeds’ view and desired admitted patient.

Complete Paediatric Clinical Observations through ED Triage document

 

Select  at the bottom of the screen, then choose ‘ED Triage’ from the list provided.

 

Alternatively, right mouse click on the patient, hover on ‘Quick Launch Documents’ and select ‘ED Triage’.

Within the ‘ED Triage’ document, on the ‘Triage’ section, scroll down to the ‘Clinical Observations’ and record any clinical observations (also known as vital signs) taken. SCM Notice is triggered if value entered is abnormal.

 

 

If ‘Level of Consciousness’ is set to ‘decreased’, SCM Notice is triggered.

The PEWS score will be triggered as a SCM Notice and ‘NEWS2/PEWS SCORE’ segment below the observation will be calculated automatically based on the clinical observations entered.

 

Select ‘Save’ once the rest of the document has been completed for the observation taken to be reflected on the tracking board view and also within the flowsheets tab.

Patient Admitted in a Paediatric Ward

From the ‘Tracking Boards’ tab, choose a paediatric ward from ‘Department’ list. Select appropriate view and desired admitted patient.

Complete Paediatric Clinical Observations through Paediatric IP Nursing Admission document

 

Select  at the bottom of the screen, then choose ‘Paediatric IP Nursing Admission’ from the list provided.

Within the ‘Paediatric IP Nursing Admission’ document, on the ‘Nursing Assessment’ section, scroll down to the ‘Objective Assessment (Observations)’ and record any clinical observations (also known as vital signs) taken.  SCM Notice is triggered if value entered is abnormal.

 

 

If ‘Level of Consciousness’ is set to ‘decreased’, SCM Notice is triggered.

 

The PEWS score will be triggered as a SCM Notice and ‘PEWS SCORE’ segment below the observation will be calculated automatically based on the clinical observations entered.

 

Select ‘Save’ once the rest of the document has been completed for the observation taken to be reflected on the tracking board view and also within the flowsheets tab.

Depending on the clinical observations entered, the information bar below ‘PEWS SCORE’ segment displays a colour from the illustration below.

Depending on the data entered, whilst saving the document, you may get an alert pop-up warning. If this occurs, read through the alert, add an acknowledgement comment from the drop-down list and select proceed to continue.

Once saved, the vital signs icon will then be displayed on the tracking board.

The vital signs icon with a green symbol shows that the patient’s observation results are at normal levels. 

 

 

The vital signs icon with a red symbol shows that the patient’s observation results have abnormal levels.

View Clinical Observation

 

The clinical observations recorded within the ‘Paediatric IP Nursing Admission’ document will now be displayed in the appropriate flowsheet document once selected.

 

With the patient in context, select ‘Flowsheets’ tab.

 

 

Within the ‘Options Panel’ on the left-hand side of the screen, the date range will show the current date (update if required) and select ‘FS Clinical Observations Paeds’ from the ‘Flowsheet Selection’.

Selected Flowsheet will be displayed in the main screen. The PEWS parameter is expanded automatically.

 

Scroll down to see clinical observations entered previously.

Complete Paediatric Clinical Observations through Flowsheets Tab

 

Please note this flow is only applicable to paediatric wards recording electronic observations.

 

  • With the patient in context, select the ‘Flowsheets’ tab. 
  • Select ‘FS Clinical Observations Paeds’ within the ‘Flowsheet Selection’ of the Options Panel.
  • PEWS parameter is expanded automatically.
  • Select  and choose ‘Add Single Time Column’.

Single Time Column triggers a pop up to record the date and time when observation was taken for patient in context. 

 

Set date and time and select ‘OK’ to see a single blank new column added to the right-hand side.

Complete each cell within the column and save.

 

Note

 flagged fields must be completed in order to save the flowsheet document.

SCM Notice

 

SCM Notice pop up notifies user based on the information entered. Read and acknowledge the notices when triggered. 

 

 

Once all  flagged cells are completed, PEWS Score pop up appears and EWS section is completed automatically.

Select  around top right-hand side of the screen. Once saved, the  on the flowsheet date column disappears.

 

Note – Whilst saving an observation, Sunrise could trigger a pop-up alert. Read the alert detail, follow appropriate clinical recommendation, select ‘Acknowledgement Comment’ from the drop-down list and select proceed.

Edit Clinical Observation

 

If a saved observation requires to be modified as a result of an incorrect entry - 

 

  • With the patient in context, select the ‘Flowsheets’ tab 
  • Select ‘FS Clinical Observations Paeds’ within the ‘Flowsheet Selection’ of the Options Panel
  • PEWS parameter is expanded automatically
  • Scroll down and select the cell of the column to be edited
  •   appears within the date and time header
  • Make changes as required

Note – Changes made could trigger ‘SCM Notice’. Acknowledge to continue.

 

  • Select  around top right-hand side of the screen

 

Note – An alert could be triggered as a result of the changes made. Read, set ‘Acknowledgement Comment’ and select ‘Proceed’

 

  •  appears at the top of the columns to indicate changes have been made.

Approving Clinical Observation

 

Some nursing staff (supervisor) will be required to approve clinical observations entered by others (supervisee) who don’t have the right to authorise their own written document.

 

The supervisor will follow the steps below once notified that the supervisee has completed the ‘FS Clinical Observations Paeds’ for the required patient.

 

  • With the patient in context, select the ‘Flowsheets’ tab 
  • Select ‘FS Clinical Observations Paeds’ within the ‘Flowsheet Selection’ of the Options Panel
  • PEWS parameter is expanded automatically
  • Scroll down and review the observations entries in the appropriate column
  • Expand 
  • Complete the section expanded and Save.

Cancel Clinical Observation

 

Note – This action is irreversible. The outcome will affect the tracking boards Vital signs symbol or PEWS information displayed.

 

To cancel an observation recorded -

 

  • With the patient in context, select the ‘Flowsheets’ tab 
  • Select ‘FS Clinical Observations Paeds’ within the ‘Flowsheet Selection’ of the Options Panel
  • PEWS parameter is expanded automatically
  • Right mouse click on the column header (with date and time) and select ‘Delete Time Column’.

 

To view cancelled observations, tick the ‘Show cancelled columns’ checkbox within the ‘Options Panel’.
