This guide is for those whose entries into the Patient Record in Sunrise need to be counter-signed, and for their Supervisors.

Counter-Signing Clinical Documents

Supervisee With the patient in context and the Clinical Document open, enter the data required.

Once you have finished, tick the ‘Incomplete’ tick box at the bottom of the document and then click on the ‘Save’ button

Inform your supervisor that you have made an entry
|

Supervisors With the patient in context, click the ‘Documents’ tab
|

With the Documents tab displayed, select the document you require from the list, checking the date. Note - the Document Status will show ‘Incomplete’ in the column
|

Click the ‘Modify’ icon from the tab toolbar
|

With the document displayed, read through the document to check the entry
|

Make any amendments if needed and, when you are happy with the entry, click the ‘Save’ button in the bottom right-hand corner of the document, ensuring that the ‘Incomplete’ tick box is unticked
The Document Status column now shows ‘Complete’

Both names will be recorded in the ‘History’ of the document. Right click on the document and select ‘View Status History’ from the menu options

With the ‘Document Status History’ screen displayed, in the box at the bottom, you will see the names of the staff who have entered into the document

Counter-Signing Flowsheets

Supervisee With the patient in context, click on the ‘Flowsheets’ tab

From the options panel on the left-hand side of the screen, select the Flowsheet you require from the Flowsheet Selection area at the bottom
|

Ensure the chart selection and date range is correct

With the flowsheet displayed in the main screen, click on the ‘+’ icon to open the required parameter
|

Add a new single time column for your data by clicking on the ‘Add Time Column Options’ icon from the tab toolbar
Select ‘Add Single Time Column’ from the menu options

Check the date and time and amend if required. Click ‘OK’
|

The new time column will appear on the Flowsheet, enter the data required by clicking into the cells
|

Once completed, click on the ‘Save’ button in the top right of the Flowsheet to save the time column entry
|

Supervisors With the patient in context, click on the ‘Flowsheets’ tab

From the options panel on the left-hand side of the screen, select the Flowsheet you require from the ‘Flowsheet Selection’ area at the bottom

With the flowsheet displayed in the main screen, click on the ‘+’ icon to open the required parameter

Read through and check the data entered in the time column
|

Make any amendments if needed in the column, and when you are happy with the entry, scroll down to the bottom of the Flowsheet. In the ‘I approve this document’ parameter (which should already be open), in the same time column, to say you agree with the data entered, insert your name, job role and the date & time
|

Click the ‘Save’ button to save the Flowsheet time column

Note – If any changes have been made by the Supervisor, when the Flowsheet is saved, a red ‘C’ icon will appear in the top right corner of the time column

Both names are recorded on the ‘History’ of the Flowsheet. Right click in the time column parameter you wish to see the History of and select ‘Show History’ from the menu options

The ‘Cell History’ pop-up screen will appear. You will see one row for each time the column has been updated and any amendments made. Use the scroll bar at the bottom to scroll across to view all information and it will show who entered the data with their name, date and time.
