The Hydration Assessment Tool guides the user to use either a Fluid Balance Chart or Hydration Chart.
Fluid Balance Chart
The Fluid Balance Chart monitors a patient's intake and output of fluid over a 24 hour period.

The Hydration Chart
The Hydration Chart is for patients who do not need a Fluid Balance Chart but need to ensure that the patient drinks enough to stay hydrated throughout the day and is passing the appropriate volume of urine.
